Day Habilitation - Individual (DHI) Services
Day Habilitation services are specified in the participants Person Centered Individual Support Plan (PCISP) and support learning and assistance in the areas of: self-care, sensory/motor development, socialization, daily living skills, communication, community living and social skills. Day Habilitation activities are intended to build relationships and natural supports.
Services are provided in a variety of settings in the community or in a facility owned or operated by an FSSA DDRS-approved provider. Settings are nonresidential and separate from an individual’s private residence or other residential living arrangements.

Individuals Served
DHI supports individuals receiving Indiana Medicaid Waiver services, including people with intellectual and developmental disabilities (I/DD) such as Autism, Down syndrome, Cerebral Palsy, and Global Developmental Delay. Services are tailored to age, abilities, goals, and personal preferences.

Decreases in stress, anxiety, boosts confidence and self-esteem, supports emotional regulation, prevents isolation, promotes movement and exercise, builds adaptive behavior skills, improves executive functions, improves attention and memory, encourages community inclusion, develops communication skills, reduces social barriers, reduces caregiver burnout, improves safety awareness, and prevents regression or institutionalization.
Documentation & Communication
We use Electronic Visit Verification (EVV) system to provide clear, person‑centered documentation to track progress and outcomes. After sessions, staff complete notes aligned to the individual’s goals and service plan. We provide updates to families and care teams as appropriate.
DHI Framework
Day Habilitation includes person-centered training, education, and support to help participants acquire and retain skills in key life areas. DHI services are always non-residential, provided in community or facility settings that are separate from a participant’s home. Sessions are typically 1:1 and may include community based time depending on the individual’s plan. Sessions commonly range from 60–120 minutes and focus on functional skill building, health and safety, and meaningful participation. Your provider will guide tasks that are appropriately challenging while promoting confidence and independence.
How & Where Services Happen
Settings: Home, community, and other locations consistent with the individual’s plan and interests.
Frequency & Duration: Based on assessed needs and authorized hours; sessions commonly 60 minutes up to 8+ hours .
Staffing: Services may be delivered individually or in a group setting. Ratio Sizes:
1:1 Individual
2:1 to 4:1 Small Group
5:1 to 10:1 Medium Group
11:1 to 16:1 Large Group (applies only to a facility setting)
One‑on‑one support by trained Pure Abilities staff; group opportunities only if appropriate and authorized.
Coordination: We collaborate with families, guardians, case managers, and other providers to ensure continuity.
Note: Service scope follows the individual’s Medicaid Waiver plan and applicable program guidance.
