Individuals must be able to care for self. Parent/caretaker should attend if additional assistance is needed.

This event is free with pre-registration but will be $5 at the door if the individual did not sign up online. Participants are not obligated but have the option for donations to the collaborative online or in person. All donations go to funding for future socials. Registration will close Thursday, October 3rd!

By Registering for this event you hereby agree to the following,
In consideration of my and/or my child(ren)'s participation in any activities affiliated with this event, Ido hereby agree to hold free from any and all liability the YMCA of Greater Fort Wayne, Turnstone, Camp Red Cedar and Easter Seals ARC and its representative officers, employees and members and do hereby for myself, my heirs, executors and administrators, waive, release and forever discharge any and all rights and claims for damages which I and/or my child(ren) may have or which may here-after accrue to me or my child(ren) arising out of or connected with our participation in any of the activities affiliated with the aforementioned organizations. I hereby do declare myself and/or my child(ren) to be physically sound to participate in the activities.

The aforementioned organizations does not furnish accident/illness medical insurance. Medical bills, including prescription drugs will be the responsibility of the participant or their parent.
I understand that registered sex offenders are not eligible to participate in or attend these activities or programs and I certify that I am not a registered sex offender. I understand the aforementioned organizations reserve the right to verify this fact against the Sex Offender Registry.
I grant the aforementioned organizations the right to take and use photographs and/or video of anyone included on this unit for print and electronic promotional material.
